Catastrophic Leave Enrollment
Certificated employees may enroll each year to the Catastrophic Leave by turning this form to the HR office by September 30.

Certificated Bereavement Leave Request
Bereavement Leave- (4) days for a death of a unit member’s immediate family. The unit member is entitled to six (6) days if traveling 300+ miles.

Job Share Application
Turn application with a Job Sharing Proposal by March 1st. The school site principal shall accept or deny the proposal no later than May 1st.
