Classified employees fill out this form to make changes to name, address, phone, and/or e-mail. 

CERTIFICATED MAY ONLY USE FOR NAME CHANGE. ALL OTHER CHANGES USE Employee Portal

This form must be filled out if an employee requests the district to release personal information to a third party. 

Resignation Form Use to request to resign or retire from your current position.

Volunteers must provide a copy of their Driver’s License or other picture ID. Specific volunteer activities that require more documents will be noted next to the activity (ex Fingerprints, TB test etc.)

Certificated employees may enroll each year to the Catastrophic Leave by turning this form to the HR office by September 30.

Bereavement Leave- (5) days for a death of a unit member’s immediate family. The unit member is entitled to six (6) days if traveling 300+ miles.

  1. Application for horizontal promotion on the salary schedule Due to HR by April 17th to be effective for the 2023-24 school year.
    2. Verified with transcripts by October 1st 
    3. Academic units completed by the first day of service if such credit is to be reflected for September 

Turn application with a Job Sharing Proposal by March 1st. The school site principal shall accept or deny the proposal no later than May 1st.
