Resignation Form Use to request to resign or retire from your current position.
Certificated employees may enroll each year to the Catastrophic Leave by turning this form to the HR office by September 30.
Bereavement Leave- (5) days for a death of a unit member’s immediate family. The unit member is entitled to six (6) days if traveling 300+ miles.
- Application for horizontal promotion on the salary schedule Due to HR by April 15th to be effective for the 2025-26 school year.

3. Academic units completed by the first day of service if such credit is to be reflected for August
Turn application with a Job Sharing Proposal by March 1st. The school site principal shall accept or deny the proposal no later than May 1st.
