Enrolling Your Middle School Student
The first step in enrolling your student at Monte Vista is completing the Pre-enrollment process.* Please click on the link below to be directed to the Pre-enrollment platform. In addition to supplying data, there are several required documents and forms that must be uploaded via the Pre-enrollment process. Please make certain you upload all of the documentation listed below before you click submit on your student's Pre-enrollment. Your student cannot be enrolled without these documents:
- Parent Photo ID
- Student Birth Certificate or Passport
- Nighttime Residency Questionnaire
- Student Health Questionnaire
- Immunizations
- Request for Student Records
- 504 or IEP (if applicable)
- Custody Documents (if applicable)
*Please note, if your student has previously attended or is currently attending a school in the Pleasant Valley School District, you DO NOT need to complete the Pre-enrollment process.
